Nauseous is the second stage of the game’s Sick moodle, and per pzwiki it appears once your internal “sickness” meter passes 50% (Queasy is the first stage, above 25%; Sick is above 75%; Fever is above 90%). The meter itself rises from one of five separate causes — zombie infection, food poisoning, tainted water, cold-and-wet exposure, or corpse proximity — and the moodle looks identical no matter which one is driving it. That is the actual problem this page solves: the symptom cannot tell those five apart, so a written answer picking one of them is guessing.

The subreddit has asked for a pinned thread about this, which is the clearest possible signal that a written answer does not exist yet. The reason is structural: “why am I dying” has at least five answers, they produce the same moodles, and every guide picks one of them and explains it beautifully. Reading three such guides leaves you knowing more and no closer.

What separates them is not the symptom. It is the test — the one observation that can only come out one way for each candidate.

Two live candidates: Zombie infection or Corpse sickness

These two produce the same moodles, so run the cheaper test first: Leave the base entirely for a full in-game day and sleep somewhere with no bodies within sight. If the queasiness recedes while you change nothing else, it was proximity.

    Burn or bury the pile, and stop dragging kills back through the doorway you sleep behind.

The ranking is a weighting, not a diagnosis. Its job is to tell you which test to run first, because running the cheap test on the likely candidate is the whole of the method. The URL carries your inputs, so a state you cannot resolve can be shared as a link rather than retyped.

The one candidate that is not like the others

Four of the five are reversible. One is not.

Rule out zombie infection first, always, and rule it out by reading the wound entry in the health panel rather than by watching moodles. The panel names the wound type, and it keeps naming it after the wound has visually closed. Moodles will never distinguish an infection from a bad meal, because the game deliberately does not let them.

Once infection is off the table, every remaining candidate is a thing you can fix this afternoon, and the correct emotional register changes completely.

The two nobody suspects

Cold and exhaustion. Soaked clothing in cold weather does steady damage that looks exactly like illness, and it comes with no illness moodle at all. New survivors read the falling health bar, assume disease, take pills, and keep walking around wet. Getting indoors, changing, and sleeping fixes it faster than anything in the medicine cabinet.

Corpse proximity. This one is invisible because the cause is a place, not an event. You cleared the door, fought there again, cleared it again, and the pile at the threshold became part of the scenery. The test is unusual in that it requires you to leave: sleep one full in-game day somewhere with no bodies in sight and change nothing else. If this test comes back positive, ranking your base by corpse load is the next step, since the doorway pile that causes it tends to rebuild itself within days.

Ordering by cost is deliberate. Four of the five resolve before you have finished reading this paragraph, which means the expensive one only has to be run when the cheap ones have all come back clean.

The version caveat that changes answers

Build 42 shifted several health behaviours, and the animal and husbandry systems introduced alongside them added new ways to be quietly unwell around a base. If you are reading advice from a Build 41 thread, the tests above still hold — they are all readings of the health panel and the inventory — but any specific numbers in those threads should be treated as Build 41 numbers until you have confirmed them yourself.
